participate in the Educational Forum of the Florida Alliance for
Diabetes Prevention and Care.
The theme of this year’s educational forum is to reach out to
community leaders to implement strategies to empower the population
living with diabetes to improve the quality of their lives.
The Forum is being brought to you without charge due to financial
support by the Palm Healthcare Foundation, Sanofi Aventis, Treasure
Coast Health Council and the Florida Department of Health Diabetes
Prevention and Control Program.
Space is limited to the first 100 participants who register so please
